*Accident Report/First Report of Injury
* Hospital Emergency Room Records, if Indicated
* Physicians Reports and/or any Operative Notes
* Reports of Lab Tests and/or X-Rays
* Physicians Treatment & Fees
* Signed Record Release From Claimant
