PROCEDURES FOR SUBMITTING A CLAIM OR RECORD REVIEW

In order to maintain the best possible relationship with your producers and insureds, we know you want to settle claims quickly and fairly. NADENT is uniquely positioned to help you accomplish just that -- and to make certain at the same time that the claim submitted is not fraudulent or overstated. If you will follow this simple CLAIM OR RECORDS REVIEW PROCEDURE, we will send you a report usually within one week of the time we receive it:

Send the following material:

THE FORM FOR CLAIM OR RECORD REVIEW (print one right from this site)
THE X-RAYS TAKEN BY THE ATTENDING DENTIST AT THE TIME OF TRAUMA
THE ATTENDING DENTIST TREATMENT PLAN & FEES*
THE FIRST REPORT OF INJURY
THE HOSPITAL EMERGENCY ROOM REPORT* (if applicable)
TMD CLAIMS: In addition to the above, a narrative from the attending dentist and any information that is pertinent to the claim from other providers (neurologists, orthopedists, chiropractors, diagnostic reports, etc.)*

*NOTE: DO NOT SEND ORIGINALS - PHOTOCOPIES ONLY!

Make sure that all of the information pertinent to the dental portion of the claim is sent to NADENT with the request. If you have any problems securing dental x-rays, make sure to tell the attending dentist they wil be reviewed by licensed dentists.

Please direct all assignments to the NADENT Regional Office for your area (the state where the treating dentist is located determines which regional office the claim should be submitted to). See the NADENT regions map for the appropriate NADENT office.
